A child was found unconscious after playing in the backyard. The doctor suspects a black widow spider bite due to symptoms like vomiting, low blood pressure, and abdominal cramps. The doctor administers an antihistamine and an antidote, and the child needs to be monitored in the hospital overnight. Blood work and a urine test will be done. Once the child is alert and eating, they can go home. I don't know doctor. My kid was playing on the backyard and I heard a noise. When I got out he was on the floor. How long was he unconscious? He was kind of asleep, kind of awake. I'm not asking what was happening. He wasn't answering. He was delirious. Did he appear to have fallen? Is there playground equipment in your backyard? Or a wall or something he could have fallen from? No, it's a small backyard. There's nothing. Do you have any toxic substances in your backyard? Any garden chemicals, cleaning products, gasoline cans? No, there's nothing. It's a small backyard, I'm telling you. There's nothing. I have all the chemicals on a safe place. Did he appear to be in pain? Was he holding himself anywhere? He was wearing his forearm. There's an injury that he didn't suffer from. Yeah, it looks like an insect bite. Yes, below the stairs on the porch. Has he vomited? Yes, he has vomited twice. His skin is kind of clammy and he appears to be going into shock. This looks like a black widow spider bite to me. We'll have to keep him under observation for the next several hours. He's a good doctor. His blood pressure is low, his temperature is down, and you can see how pale he is. I'm going to put him on an IV. Well, a black widow spider bite can be very serious. It comes on the patient because of what we call anaphylactic shock. I think we've cut it short. He appears to be having cramps and his abdomen is rigid. He's having some trouble breathing. He has some chronic symptoms. What does the injection have? It's a glucose solution with adrenaline and some other drugs to bring him out of the shock. Keep him hydrated and so on. It's also an analgesic for the pain. Yes, very good. I'm going to put him on a cardiac monitor too, just in case. How much time does he have to be here? We'll check his vital signs every couple of hours to see how he's doing. I can't tell you for sure. This is where I left my kids with the neighbor, so I have to tell her. Well, I really can't tell you. We can't release him until his blood pressure is back to normal. He's breathing normally and he doesn't have any other pain. Of course. I'd say he'll probably have to be admitted and spend the night in the hospital, just so we can keep an eye on him.
