You will also receive a delivery note which is to be filled out when all equipment is received at the site. And a return note which is to be filled out at the end of the study when all the equipment is ready to be shipped back to Quantificare. Please consistently use a room without windows. If the room has windows, close the blinds and use artificial lights to ensure similar lighting conditions at each visit. Please also ask the subject to remove any jewelry, clothing, bandages or hair from the area being photographed. To complete the photo session, you will need to capture the set of four photos. Upload these photos online by accessing our web platform, app.dermapix.com. And wait for the project manager at Quantificare to perform the quality control on each of these images. And respond to any queries if they are issued. Upon receiving the camera before each photo session, please check the camera settings to ensure they have not changed. To do so, turn on the camera. Check the settings by comparing the settings on the back screen of the camera to those provided in the image procedure or camera manual. If the settings are different, please refer to the camera manual to correct them or call your point of contact at Quantificare. When operating the camera, you will need to complete the following steps. Turn on the camera by turning the on switch to the right. Enable the live view function by pulling the LV trigger towards you. The two red beams are automatically displayed. Interpose the beams by moving the camera forward and backwards until the beams perfectly overlap. Then, press the button fully to capture the image. To prepare for a photo session, please verify that the SD card is correctly inserted and the battery is charged and inside of the camera. Prepare the subject for the photography session by asking them to tie up their hair if they have it down and put the headband on. The headband should be placed high enough to keep the forehead, hairline, and ears fully visible. Then, instruct the subject to remove any jewelry, makeup, bandages, glasses, or piercings from the area being photographed. Finally, assist the subject with placing the black neck collar around their clothing to provide uniformity across all visits. A photo session will begin by filling out the ID card. Please note the study code will be pre-filled for you, but you will need to fill in the rest of the information with a thick black pen. The subject code, the visit code, and the visit date. Next, take a picture of the ID card with the camera provided. Move the camera back and forth until the beams are superimposed and then fully press the button to take the picture. The four set of photos will be used to perform a full 3D reconstruction of the subject's face. These photos include and must be taken in this order at every visit. The front view, the 45 degree under right view, the 45 degree under left view, and the neck view. The first photo in the set of four photos is the front view. To take this photo, position the subject in front of the uniform blue background. Ask them to look straight in front of them with a neutral facial expression. Remind them they should keep the same position throughout the photo session and they should not incline or raise their head. Position yourself in front of the subject. Place the camera at the height of the subject's head between the nose and forehead to keep the camera parallel with the subject's face. Superimpose the dual beams on the tip of the nose. Always look at the right side of the display screen while framing the subject's face. Look at the display screen to make sure the subject's face is centered and that the ears are seen, the eyes are open, and the mouth is fully closed. Then, press the button fully to take the photo. The second and third photos in the set of four photos are the 45 degree under right view and the 45 degree under left view. After the front view, always continue with the 45 degree under right view photo. To take this photo, from the front view position, take one step to the left and turn slightly towards the subject. Position the camera lower at the level of your chest and incline it upwards. Superimpose the two red beams on the point of the cheek in the zone between the cheekbone and the corner of the mouth. Then, press the button fully to take the photo. To obtain the 45 degree under left view, take two steps to the right and repeat the same procedure. Position the camera at the level of your chest and incline it upwards so that the neck area below the jawline is fully captured. Superimpose the two red beams on the point of the cheek in the zone between the cheekbone and the corner of the mouth. Then, press the button fully to take the photo. The fourth photo in the set of four photos is the neck view. After the 45 degree under right and left view photos, always continue with the neck view. To take this photo, position yourself in front of the subject. Straighten your arms and position the camera below the level of your chest and incline it upwards. Superimpose the two red beams on the center of the chin. Then, press the button fully to take the photo. Check the quality of the four photo set. Press the display button on the camera. Make sure that the head is not cut off and the photos are correctly framed. On the 45 degree under left and right photographs, the neck area below the jawline must be fully visible without any shadows or hidden regions that will create holes on the 3D reconstruction. Retake the whole set of photographs if necessary. An account is created for each clinical staff member participating in the photography procedure. In order to upload these images into the Quantificare web portal, you will need to submit your electronic signature letter. Complete and sign the electronic signature letter that was provided to you with the emailed login information. This will need to be scanned and uploaded back into your profile on the server. Now, we are going to upload the images. Remove the SD card from the camera by pressing on it to release it. Put the SD card in the USB SD card reader and plug it into the computer. Now, you will need to access our secure website, www.app.dermapix.com. Type in your account name and your one-time password and click on Sign In. On your first connection, you will be asked to personalize your password. Once complete, click Update Password. On the home page, click on the study code titled EBO-CLIN-202 displayed on the Studies tab. Then, click on Site Code. To create a new subject, click on the plus sign icon in the right-hand corner. Enter the subject code. For type, you will select several options. Real, Test, or Qualification. Real is used for actual subjects in the study. Test is used for uploading test images if you would like to practice. And Qualification is used for uploading your qualification images. Once a new patient is created, a new line will appear. Click on the subject code you just created. A new patient images grid will display. Click on Edit Grid. Then, on the Upload ribbon below, click on Upload to find your pictures and select them. You can use the Shift button to select all the pictures at once. Then, click on Open. When your images have been uploaded, they will appear in the Upload ribbon. You can simply drag and drop them to the correct location. After you drag and drop your first images, the date of the image will appear just below the visit code. If the date is not correct, you can modify it by clicking on the visit date. The calendar pops open and you can select the correct date. Quantificator will perform the masking on the images to ensure that the images are all de-identified. When you're finished, click on Save Grid. Enter your password and click on Save. A pop-up window will close itself and the photographs will be added to the location on the grid. They will be saved on the server. There is an option to leave a comment if necessary, but you do not have to. Unless if you've made some modifications, changed the visit date, or deleted photos from a previous visit. Do not forget to sign out and do not delete the images from your SD card. If you lose your password, you can request it on this page.
