Lack of staff training and also parts of the multidisciplinary team coming round to see the patient and not communicating what's to be done next with the patient and how things are changing to be done. Season 2022. What matters to you is an excellent foundation for care planning. It might lead to misrepresentation of people's preference. This should be greater emphasised on how the patient's mental and physical condition is in an acute medical setting. This is direct influence on how detailed and or accurate the initial interviewing results are. The example for our frequently observed was that no one had a what matters to me on their board at their bedside. I know that, in my place, an acute medical receiving award means that it's for short stay for patients, but to have remarks helps the staff to have some sort of information to the person, what they like to be called, how they mobilise and it helps to start a conversation with the patient. NHS 2024. According to the World Health Organization, 2024 health promotion is a process of enabling people to achieve control over and to improve their own health. The word health is defined as a state of complete physical, mental and social wellbeing and not merely the absence of disease or infirmity. In my placement, I first experienced having discussion about the patient's lifestyle choices as we were the first place we came to and we'd fill in their admission packs. In the conversation, I asked them a variety of questions, a variety of lifestyle questions. This would include, do they smoke or drink alcohol? Have they taken any recreational drugs before? How do they mobilise? Are they in contact of urinary faeces? Do they raise a hand for personal care and about their past medical history? Having a conversation helps to open the dialogue about maybe looking at a better lifestyle change for them and to get them the correct information regarding their choice. While in this placement, I was tasked to admit several patients. I found that at this point of conversations between me and the patient was that when I could talk about promoting positive lifestyle changes to them, most of the patients that were smoking in particular, the National Institute of Health and Care Excellence 2024, I found that having these conversations helped me use basic motivational interview strategies such as Active Listen 2024. I found that this gave positive effects to the patient to receive the help they needed to go forward. When having a conversation regarding the smoking, I refrained from my own judgment and didn't lecture the patient. This was the way that I spoke to the patients regarding their drinking habits. Speaking to them, not lecturing them, this was a massive help as I asked them, have you tried to cut back or quit before? This technique, I found, had best success for the patient and many went on to nicotine replacement therapeutic support as well as the patient who drank alcohol a lot went on to help groups and further help were available within the committee through 2024. I found that having normal discussions with the patients was beneficial for me as a student nurse because the patients felt more at ease with me rather than being assessed by the mock disability team. The NHS promotes this flexible approach to habit change which is known as making every contact count. This approach encourages all members of healthcare staff to use everyday interventions to promote and discuss the patient's ways to support their physical and emotional wellbeing and is an important component of most personal holistic approach to healthcare. NHS 2023. These are numerous barriers to successful health talks including a lack of staff training and understanding of a patient's social determinants of health such as cultural, educational and social needs in our post-status as well as the access to suitable housing and healthcare. A change I would make in the work would be more information given to the patient regarding their illness. Patients require education on the disease. They must be aware of their conditions, understand their symptoms and approach and self-help administering their drugs and taught them to seek medical help. To improve a patient's physical and emotional wellbeing, personalised patient education materials in addition to verbal instructions by multidisciplinary teams contribute to better patient care to ensure decision making and increase patient satisfaction. It is important to emphasise that multidisciplinary teams understand their patients' concerns and provide appropriate education and counselling to help optimal healthcare delivery. Common to Rutherglen-Glantz 2024, the health belief model is a theological framework that can be used to direct health promotion and illness prevention effects. Its purpose is to explore and predict individuals' changes in health behaviours. The health belief model's key components centre on individuals' beliefs about their health problems which predict individuals' health-related behaviours. The model defines the key factors that influence health behaviours and individuals' perceived threats to sickness, disease, belief or consequences, positive benefits or actions, perceived barriers to actions and exposure to factors that prompt action and succeed. Using this theory in the world would be beneficial to patients to let them move and walk, to be more involved in their care and being able to make changes when needed, help them cope better as they know more information regarding an illness as a whole. Another health promotion theory is the transversal stages of change model. That was developed and researched by James Protosty and Carlo D'Alament in the late 1970s. The theory focuses on processes that individuals go through when making behavioural changes. Those stages include the pre-contemplation, preparation, action and maintenance. According to Lamorne 2020, critics of the model said that, ignorant of the social context in which change happens, such as the psychonomic state in turn, decisions between the stages can be subjective, with no clear criteria for determining a person's stage of transformation. There is no clear understanding of how much time it requires at each stage of how long a person stays and remains in one. The concept supposes that individuals make logical decisions that are not always the case. The theory is designed to reduce people's behaviours and lack of prevention, whereas the health belief model is more suited to my model, to promote more involvement of change for good. According to the Department of Health and Social Care 2020, two thirds of the Scottish population are overweight, including obesity. That is a major public health concern. According to the Department of Health and Social Care 2020, avoiding obesity as a risk factor may result in cardiovascular disease, type 2 diabetes and obesity-related cancers, which generate much more morbidity and years of loss associated with increased levels of obesity. Obesity has a clearly miserable impact on the physical and mental health of populations, as well as considerable direct and indirect costs. The evolving obesity pandemic is an exceptional and considerable toll on those affected. The treatment and health service in our communities Willer initially described his motivation for interviewing me in 1983, when he was conducting research on alcoholism therapy. A term was widely caught on throughout the healthcare system as a way to help patients to practice health promotion strategies, such as the personal recommendations for diet change or healthy behaviour, such as incorporating exercise into a daily routine, by ensuring that the patient realises for themselves what they have the power to make and recognise the problem and recognise how to deal with the problem and feel motivated with a plan going forward to implement it. Given that a patient's counselling is not enough, a plan for how the information is taken in and how the patient thinks is put into their day-to-day living. According to the Scottish Government 2020-23, the proportion of adults who are overweight or obese has increased as areas of deprivation increased. The proportion of those with overweight increased from 59 per cent in the least deprived areas to 71 per cent in the most deprived areas. The proportion of adults living with obesity increased from 19 per cent in the least deprived areas to 26 per cent in the most deprived areas. Another behavioural change technique is self-monitoring, which involves observing and recording a food activity routine, followed by feedback on their behaviours. The purpose of self-monitoring is to promote self-awareness in target behaviours and outcomes, so that it can act as an early warning system if a problem is emerging. It allows the person to see what and where has gone wrong, and it can help to establish where the problem is stemming from. I found my placement in a great working environment, and my research for the presentation has given me a lot of good information going forward into my next placement. However, I have noticed that a lack of resources or staff and patients are a big problem, and that it is going to get worse for the care and patients in a safe environment if the problem goes on as soon as they are diagnosed.
